Information You Choose to Send Us
This website has three forms: Contact, Refer a Client and the Recovery Housing Application. Depending on the form, we ask for:
- Your name, phone number and email address
- How and when you prefer to be contacted, and whether we may leave a voicemail
- The type of service you are asking about
- For referrals, your organization and role
- For housing applications, your current city, requested move-in date, current living situation and how you heard about us
- Any short message you choose to include
Please do not include diagnoses, treatment history or other detailed health information in a website form or a regular email. If we need that information, we will collect it through a secure intake process.
Information Collected Automatically
This website does not use advertising cookies or analytics tracking, and it does not follow you across other websites.
Like most websites, the company that hosts our site may keep standard server logs, such as IP address, browser type, the pages requested and the date and time of a visit. These logs are used to keep the website secure and working.
Our pages load typefaces from Google Fonts. When a page loads, your browser connects to Google's servers, which receive your IP address. Google's handling of that information is described in Google's privacy policy.
How We Use Information
We use the information you send us only to:
- Respond to your question or request
- Explain services and arrange the appropriate intake, screening or assessment
- Process professional referrals and recovery housing applications
- Keep this website secure and working
- Meet our legal and regulatory obligations
How We Protect Information
Information received through this website is handled under the same confidentiality policies that apply to client information. Access is role-based and limited to staff who need it, only the minimum necessary information is used, and every workforce member completes confidentiality training before working independently and every year after that.
Email and Text Messages
Regular email and text messages are not fully secure. For that reason:
- Our staff use only authorized work email accounts and do not put sensitive clinical details in email subject lines.
- Text messages are used only for appointment reminders, scheduling and limited administrative communication, and only after you agree to receive them.
- We will not send assessments, diagnoses or detailed behavioral health information by text message.
When We Share Information
We do not sell personal information. We share information only when the law allows it, for example:
- With your valid written authorization
- To coordinate the services you asked about
- For payment or the operation of our services
- When required by law, such as a court order or a mandatory report
- In an emergency involving a risk of harm, as the law permits
Companies that help us run this website, such as our web host or a form service, may process information on our behalf and are expected to keep it confidential.

You may ask us to update or delete information you sent through this website. If that information has become part of a client record, it is kept and destroyed according to our record retention requirements and the rights described in our Notice of Privacy Practices.
