If our site is not fully accessible for your needs, we will talk with you before your first appointment and arrange a reasonable accommodation, including a referral to an accessible provider when needed.

We record only what staff need to know to help you, and we keep disability-related information confidential.

Getting to Our Office
Our office is at 2851 S Parker Rd, Ste 640, Aurora, CO 80014. If you have mobility limitations, tell us when you schedule and we will:
- Explain where accessible parking is located
- Give you the best route to the accessible entrance
- Offer curbside assistance when it is available and appropriate
We keep hallways and doorways clear for wheelchairs and other mobility devices and check them every quarter. If an accessible restroom is not inside our suite, staff will give you clear directions to the nearest accessible restroom in the building.
Language Access
If you prefer a language other than English, we ask about it at intake and arrange an interpreter when feasible. We do not ask family members to interpret clinical information unless you request it and it is appropriate.
How to Request an Accommodation
- Ask any staff member, by phone, by email or in writing.
- Staff notify our Administrator or Accessibility Coordinator.
- We review your request and discuss options with you.
- We put the accommodation in place promptly.
- If we cannot provide the accommodation at our site, we offer a timely referral to an accessible setting or provider.
